  • Have your GP or medical specialist complete the medical certificate portion of the form
  • Send the following items to the CTP insurer of the driver who is at fault in the accident as soon possible:
      • Completed form
      • Medical reports
      • Documents supporting your claims
      • Accounts and receipts for expenses related to the accident

CTP claims and car accident compensation claims can be very complicated and time-sensitive. It is important to stress that if you accidentally miss even one deadline, you may never receive compensation for your injuries.
